Additional Details:

Preferred Skills/Experience: 2 years of recent home care or hospice experience including venipuncture, catheterization, wound care, and/or parenteral therapy preferred.

Required Credentials: Valid driver's license and appropriate auto insurance required.

Unit Details: Provides skilled nursing, admissions, and case management for patients in their place of residence; coordinates continuity of care through an interdisciplinary team approach. Includes patient/family instruction, symptom management, oversight of Home Health Aides and CNAs, OASIS documentation, telephone triage, and home visits as needed. May start as a Visiting RN and move to Case Manager; the same clinical responsibilities apply.

Special Requests: a valid driver’s license, appropriate auto insurance, and reliable transportation required.

Shift & Scheduling: Mon-Fri. On-call and weekend rotation. Holidays as required.
