Unit Details: Screens, evaluates, re-evaluates, and treats patients; coordinates and communicates treatment activities with interdisciplinary team members and the patient. Develops a care plan in collaboration with the patient's physician, the patient, and their family or primary caregivers upon admission; periodically reviews and recertifies the patient care plan as appropriate. Coordinates the patient's care plan through interdisciplinary communication and case conferences as appropriate and advises other disciplines of discharges in a timely manner. Provides ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals and communicates significant changes to the patient's physician and VNH colleagues. Documents evaluations, treatment goals, and objectives. Teaches the patient and caregiver self-care techniques as appropriate and provides medication, diet, and other instructions as ordered by the physician, using opportunities for health counseling with patients and caregivers. Coverage areas include Bradford, VT; Randolph, VT; Springfield, VT; White River Junction, VT; and Lebanon, NH.

Unit/Facility Details: Home Health

Required Credentials: Graduate of an accredited school of nursing.

Required Skills/Experience: 2 years of experience in Home Health.
