Special Requests:
- Previous travelers who have worked at Bassett for 1 year must take a 9-month break before returning as a traveler.
Preferred Skills/Experience:
- Previous home dialysis experience is strongly preferred.
- Peritoneal dialysis (PD) experience is highly desirable.
- Experience educating and training dialysis patients is preferred.
Unit Details:
- Outpatient home dialysis role supporting approximately 14 home dialysis patients across the network.
- Primary patient population is peritoneal dialysis (PD); program supports both peritoneal dialysis (PD) and home hemodialysis (HHD).
- Manage outpatient home dialysis clinics, conduct patient assessments and ongoing clinical management, provide modality education and training for new home dialysis patients, troubleshoot and triage patient concerns and treatment-related issues, coordinate care, and support patient independence in the home setting.
- Complete initial home visits for new patients as needed; ongoing patient care is primarily provided in the clinic setting.
Shift & Scheduling:
- Weekday call coverage: 4:30 PM – 6:00 AM.
- Weekend call coverage: 24-hour coverage Saturday and Sunday.
- Call rotation is typically every other week.
Floating Requirements:
- Travel between clinic locations in Cooperstown, Oneonta, and Little Falls may be required; RNs are only expected to travel to patient homes for the initial home assessment/visit; personal vehicle required.
- Occasionally assist inpatient nursing units with CAPD exchanges.
