Additional Details:
An interview may not be available prior to offer. At least one reference must be from the same unit/specialty as the job you're applying to.
Required Skills/Experience: Experience with discharge planning and utilization review.
Preferred Skills/Experience: Strong discharge planning background for heavily socially impacted discharges preferred. Familiarity with placement agencies, Medicare/Medi-Cal guidelines as they pertain to SNF and lower level placement, and strategies to discharge individuals who are not heavily resourced preferred.
Special Requests: Cannot have worked at any CommonSpirit location within the last 12 months. Cannot apply to more than one job order on the same unit and shift at the facility. Prefer minimal time off requests.
Unit Details: Uses Midas, InterCall, Curaspan, and Microsoft Office. Works through heavily socially impacted discharges such as room and board/board and care placements, conservatorship, and under- or uninsured cases.
Shift & Scheduling: Every other weekend. No overtime.
Floating Requirements: First to float and must be able to cover all adult areas of the hospital. If no women's health experience, the traveler will not cover this.
