Required Skills/Experience: Will consider someone with strong Med Surg experience with some Home Health experience.

Preferred Skills/Experience: OASIS and start of care experience strongly preferred.

Required Credentials: Valid driver's license and car insurance required.

Unit Details: Performs home visits and some telehealth/virtual visits. Territory within 15 miles; may increase if census is low. 24-26 caseload; if given a bigger territory that number will go down. Completes OASIS data. Services include catheter care, chronic disease management and telemonitoring, diabetes management, home health aide services, infusion management (including antibiotic therapy), laboratory testing, medical social service, nutrition instruction, physical, occupational, and speech therapy, skilled nursing, telemonitoring and virtual visits, tube feeding, and wound/ostomy care. Provided an iPad.

Shift & Scheduling: Mon-Fri. No weekends. No call.
