Required Skills / Experience:

• Current Home Health experience

Preferred Skills/Experience:

• Previous hospice experience preferred.
• 2-5 years of med-surg experience preferred.

Required Credentials:

• Valid driver's license and current auto insurance coverage required.

Unit Details:

• Create a tailored plan of care for both Home Health and Hospice patients and care for patients at all stages of life in their homes.
• Home care services include skilled observation and assessment, nursing care management, disease management, surgical aftercare, wound care, injections and IV therapy, maternal and baby health, pediatric care, rehabilitative services, and therapeutic assessment and rehabilitation planning in the home.
• Palliative and hospice services include in-home consultation, advanced care planning, symptom management recommendations, certified hospice nursing assessment, in-home provider visits for pain and symptom management, and bereavement and grief support.

Shift & Scheduling:

• Monday-Friday.
• No weekends.
• No call.
• Flexible workday with option to start and end the day from home.

Floating Requirement:

• Territory: South Territory - Littleton/Plymouth areas
