Additional Details:
All positions at Providence Health & Services require any lab items requested during onboarding to be completed within 72 hours of signing a contract, no exceptions. Candidates must be responsive and willing to go to a lab ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility. References must cover at least 1 full year of relevant specialty experience within the past 3 years. You must be able to complete labs within 48 hours of signing. ER profiles must include the size of the facility/number of beds and the trauma level designation, if any. Orientation for all Oregon facilities is in Portland, OR for RNs, LPNs, and CNAs.
Required Skills/Experience:
- 1 year of skilled home health experience, OASIS experience required.
- First-time travelers not accepted.
Required Credentials:
- Cannot have any hits/suspensions on license or any background hits from any point in the past.
Special Requests:
- RTO must be listed at time of submission.
- Cannot be currently or recently (within 6 months) employed with the network. Former employees must have left in good standing.
Unit Details:
- Covers Salem, Keizer, Woodburn, Molalla, and surrounding communities.
- May visit up to 6 patients per day; 30 productivity points standard.
- Adult and geriatric patient population.
- Clients are generally grouped to minimize drive time; occasional maximum radius of 50+ miles.
Shift & Scheduling:
- May need to work some weekends.
- No on-call required.
