Unit Details:

• Home care case management with OASIS documentation.
• High-tech home health skills include IV use and maintenance, wound VAC, and complex wound care.
• Coordinates interdisciplinary care with physicians, providers, team members, vendors, physical therapy, and social work; supervises HCA services.

Shift & Scheduling:

• Tuesday-Saturday, 8am-4pm
• On-call rotation required; hospice coverage required when on-call.
• Holidays as required.
