Unit Details:
Serves as case manager for assigned patients and actively manages caseloads and visit times.
Completes OASIS documentation in accordance with agency policies and applicable state and federal regulations.
High-tech skills include IV use and maintenance, wound VAC, and complex wound care.
Works collaboratively with LPNs, delegates tasks, and provides ongoing supervision for HCA services.

Shift & Scheduling:
Monday-Friday schedule with some weekends, on-call, and holidays required.
Hospice coverage is required when on-call.
