Required Skills/Experience: 1-2 years current Home Health and Hospice Experience Homecare experience a must

Required Credentials: Associate's Degree or Certificate

Preferred Credentials: Bachelor's Degree

Unit/Facility Details: adults 18yrs and older, chronic disease management including CHF, COPD, Diabetes, Cancer, CVA, wound care, IV administration

Special Requests: Needs to be able to adapt and be flexible, adjust to the ever-changing schedule that is home health and hospice, timely documentation, good solid skill set and assessment abilities, knowledgeable about home health OASIS and hospice. Able to hit the ground running
