Required Credentials:

Required Skills/Experience:

• 1 year Certified Home Care experience.
• 1 year Case Management experience.

Unit Details:

• Ability to travel to cases as assigned.
• Coverage area includes Back Bay, Beacon Hill, Boston, Cambridge, Charlestown, Chinatown, Fenway, Financial District, Kenmore, North End, Prudential, South Boston, South End, and West End.
• Home Health RN responsibilities include assessing patient conditions, implementing care plans, providing ongoing education and support to patients and their families, completing admissions, performing case management responsibilities, and utilizing thorough and timely electronic documentation and interdisciplinary communication.

Shift & Scheduling:

• Weekend rotations are once every 6 weeks.
