Required Skills/Experience: 1 year Certified Home Care experience and 1 year Case Management experience. Experience in hospice, private care, or working as a Case Manager on its own does not meet the qualifications for Certified Home Health Care Management experience. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement.

Required Credentials: Contingent/temp licensure is not accepted.

Special Requests: Ability to travel to cases as assigned.

Unit Details: Responsibilities include assessing patient conditions, implementing care plans, providing ongoing education and support to patients and families, performing admissions and case management responsibilities, and utilizing thorough and timely electronic documentation and interdisciplinary communications. Covers Back Bay, Beacon Hill, Boston, Cambridge, Charlestown, Chinatown, Fenway, Financial District, Kenmore, North End, Prudential, South Boston, South End, and West End.

Shift & Scheduling: Weekend rotation is once every 6 weeks.
