Required Skills/Experience:

• Must have two (2) years of experience as RN in home hospice environment.
• Experience using an EMR.

Preferred Skills/Experience:

• Three (3)+ years of RN experience.

Required Credentials:

• Associate's (ASN) degree
• OR
• Diploma with RN License in New Jersey
• Must have valid New Jersey driver's license.

Unit Details:

• Home hospice case management with patient/family assessments, plan of care development, patient/family education, medication reconciliation/management, interdisciplinary team communication, and HHA supervision.
• Coverage includes Morris and Sussex Counties.

Shift & Scheduling:

• Weekend requirement every 3 weeks; comp days provided.
• One summer/winter holiday.
• No on-call.
