Required Skills/Experience:
Two (2) years or more of discharge planning experience required.

Special Requests:
Flu declinations accepted on condition traveler wears a mask at all times.
Housing is limited.

Unit Details:
Performs case management and coordinates discharge planning for age-appropriate inpatients with chronic diseases and special health care needs across the Multi-Service, Intensive Care, OB, and Adolescent Care units.
Maintains a tracking system for patients referred for admission to other healthcare facilities, performs chart reviews for requested services, and coordinates return to the facility or discharge home.

Shift & Scheduling:
Monday through Friday.
15-minute call response time for all units with a call requirement.
