Additional Information
Required Credentials:
- Valid driver's license, reliable transportation, and auto insurance required.
Preferred Skills/Experience:
- Home health experience preferred, but not required.
Unit Details:
- In-person home health role providing skilled, patient-centered care in the community.
- Independent visits in patient homes with flexibility in how you structure your day.
- Performs comprehensive patient assessments, develops and updates individualized plans of care, provides skilled nursing care and interventions, monitors patient progress and reports changes to the care team, educates patients and families on medications, treatments, and care plans, coordinates care with LPNs, therapists, caregivers, and providers, and completes timely, accurate EMR documentation.
Shift & Scheduling:
- Monday-Friday schedule.
- On-call rotation required.
