Additional Details:
Required Skills/Experience:
- Minimum of one year of experience working with the frail, elderly, or long-term care population.
Preferred Skills/Experience:
- Minimum of one year home care experience preferred.
- Minimum of one-year wound care experience preferred.
Required Credentials:
- RN–Nursing Diploma or Degree from accredited School of Nursing or University.
Preferred Credentials:
- Bachelor's degree in science of nursing preferred.
Unit Details:
- Clinic and home-based direct care for participants.
- Triages emergent situations and monitors acute and chronic medical conditions in coordination with primary care providers.
- Regular home visits; wound care, lab draws, nurse assessments, treatments, procedures, and medications as prescribed.
- Uses TruChart.
Shift & Scheduling:
- Mon-Fri from 8:00 - 4:30
- May work beyond normal working hours and on weekends and holidays, as necessary.
- Flexible contract hours: 40 per week.
