Additional Details:
Required Skills/Experience:
- 1 year of experience working with the frail, elderly, or long-term care population.
Preferred Skills/Experience:
- 1 year of wound care experience preferred.
- Home Care Experience
- TruChart Experience
Preferred Credentials:
- BSN preferred.
Unit Details:
- Field-based home visits with in-person participant assessments to determine the appropriate level of care and respond to calls from PCPs, triage, and support staff.
- Care may include wound care, IV hydration, lab draws, nurse assessments, emergency treatment initiation, and routine, acute, and non-acute visits.
- Serves frail, elderly, and long-term care participants.
- Coordinates with providers, DME suppliers, ALF, SNF, paramedics, pharmacy, home care coordinator, and hospital partners.
- Uses TruChart.
- Must have reliable transportation, current valid Michigan driver’s license and car insurance
Shift & Scheduling:
- Saturday and Sunday schedule, 7:00am-11:00pm
- Holiday Coverage: No
- No Call Requirement
