Required Credentials:

• Dementia testing will be required prior to start.

Required Skills/Experience:

• 1 year of experience within the past 3 years must be within a Skilled Nursing Facility.

Special Requests:

• Must take full 6 months off from any Sanford Health and Marshfield Clinic Health System location as well.
• Must disclose previous employment on your resume or elsewhere within the submission for a DNU check.

Unit Details:

• Ratios: 2 nurses (typically 1 LPN-12 hour shift and 1 certified medication aide-10 hour shift) per 48 residents; 1 LPN at noc. 1 RN unit manager also present during day 8-10 hour shift.
• Common diagnosis/types of patients: Dementia, heart disease, COPD, CKD, depression.

Shift & Scheduling:

• Every other weekend and every other holiday.
• No on-call or call back.
