Required Skills/Experience: 2 years acute case management experience.

Unit Details: Provides case management services for inpatient and observation patients, including comprehensive psychosocial, medical, and discharge assessments; coordinates the plan of care and drives discharge planning with the multidisciplinary team; evaluates progression of care using InterQual throughout the episode of care. Uses MIDAS, Careport, and InterQual.

Shift & Scheduling: No call required. Rotating weekends.
